Client’s documents for opening of deposit account

In order to open a deposit account, it is necessary to provide the following  documents:

  • a copy of a constituent document (notarized or certified by a registration authority);
  • a copy of order on appointment of chief executive officer empowered to act on behalf of the organization without power of attorney (certified with a signature of the chief executive officer and the enterprise’s seal);
  • copies of the documents confirming powers of the individual who will sign a deposit agreement and additional agreements thereto – only for the individuals, whose powers are not determined by the constituent documents (certified with the signature of the chief executive officer and the enterprise’s seal);
  • a copy of certificate of the State Statistics Service of Ukraine on inclusion into the Unified State Register of Enterprises and Organizations of Ukraine (notarized or certified by the registry authority, or with the signature of an authorized employee of Bank); 
  • a copy of certificate from the State Tax Inspectorate of Ukraine on registering a taxpayer – form No. 4-ОПП (notarized or certified by an issuing authority, or with the  signature of an authorized employee of the Bank);
  • a copy of certificate on state registration in executive authority (notarized or certified by the issuing authority, or by signature of the authorized employee of Bank);
  • a letter informing the Fund of Social Insurance Agains Industrial Accidents and Occupational Diseases of Ukraine about an intent to open a deposit account in “PRAVEX BANK” JSC (MFO 380838, address: 9/2 Klovskyi Descent, Kyiv, 01021), only for legal entities employing hired labor, with the mark of the Fund on receipt of the letter (with the signature of the chief executive officer and the enterprise’s seal); 
  • letter containing the following information: details of the  account, to which the deposit amount and interest accrued thereon will be paid back,- a number of the account, name of the bank, its MFO (if a foreign currency is being transferred, additionally the details of intermediary bank), actual (mail) address (with the signature of the chief executive officer and the enterprise’s seal);  
  • Enterprise’s balance sheet (Form No. 1), P&L statement (Form No. 2) as of the last reporting date (with the signature of the chief executive officer and the enterprise’s seal);
  • letter containing information that allows to carry out identification of individuals owning the legal entity and holding 10 and more percent of the registered capital exerting direct or indirect influence on and obtaining economic gain from the enterprise’s activity. The letter shall in regard to each individual specify the share of registered capital owned by respective individual. The letter shall be signed by head of the enterprise (his/her deputy, legal entity’s seal should be affixed (according to the requirements of article 64 of Law of Ukraine “On Banks and Banking Activity”, article 18 of Law of Ukraine “On Financial Services and State Regulation of Financial Services Markets”)).

Furthermore, the individuals who are beneficial owners shall mandatorily  specify: 

For residents:

  • First name, surname, patronymic.
  • Date of birth.
  • Series and number of passport, date of issue and issuing authority.
  • Residence (registration place).
  • Taxpayer identification number.

For non-residents:

  • First name, surname, patronymic (if any).
  • Date of birth.
  • Series and number of passport, date of issue and issuing authority.
  • Citizenship.
  • Place of residence or place of temporary stay.
