Documents confirming payments from the social protection authorities
While opening a pension account, you need to present the following documents:
- a pension certificate or certificate from the Pension Fund of Ukraine confirming that you are the recipient of pension payments.
While opening an account to receive payments connected with the birth of a child and care of him, you'll need to provide one of the following documents:
- a medical certificate confirming maternity leave before the birth of a child;
- birth certificate of the child;
- certificate from a social protection authority confirming that the documents for allowance have been submitted.
While opening an account to receive payments connected with the Chornobyl disaster liquidation, you'll need to provide one of the following documents:
- certificate of a sufferer of the consequences of the Chornobyl disaster;
- a certificate from a social protection authority confirming that the allowance of the Chornobyl disaster sufferer is assigned.
While opening an account to receive an unemployment allowance:
- a labor book, where the last record is the dismissal record from the last job, or an employment center registration mark.
While opening an account to receive another type of social allowance:
- a certificate from a social protection body for the assignment of any other type of allowance.
